However, MSCs can do more for the development of cell and gene therapies. With the latest research, our understanding of the mechanisms induced by MSCs in therapeutic applications has shifted. MSCs also provide therapeutic benefits in the host body by a pair of crune effects and the stimulation of host cells, as they can modulate the innate and adaptive immune responses. Indeed, MSCs not only serve as tissue-progenitor cells, but they also have the ability to alter the tissue environment. How does this work to result in a therapeutic potential? MSCs can mediate immunosuppressive responses thanks to the immunomodulation of monocytes or macrophages, dendritic cells, T cells, B cells, and natural killer cells. As such, MSCs can either directly modify the environment by producing cytokines, chemokines and growth factors, or modify the functions of the microenvironment through the release of extracellular vesicles. These immunomodulatory functions are already helping to develop therapies. Let's take a look at a practical example. Allogeneic MSCs are extracted from donors. These allogeneic MSCs can activate the T cells from the patient and thus be used to develop CAR-T cell therapies. In another example, MSC derived extracellular vesicles are used in clinical trials to treat different diseases, such as severe and critically ill COVID-19 patients. These immunomodulatory functions provide a promising role for the development of more effective cell-engine therapies in treating inflammatory, autoimmune, or cardiovascular diseases, as well as cancer.
